1: The ER

In a vision-threatening emergency, the ER is likely to be your first port of call. Some reasons that you might head straight here include:

  • Penetrating eye trauma (AKA, a sharp object in the eye).
  • Blunt eye trauma (such as from a punch or any other large object hitting you in or around the eye).
  • Head trauma that affects the eyes, sockets, or bones near the eye/s.
  • Retinal detachment (symptoms include seeing flashing lights, the appearance of new floaters, and a dark veil or curtain protruding down from the top or outside corner of your eye).
  • Sudden vision loss in one or both eyes.
  • Sudden onset of any vision changes—such as severe blurring or severe double vision.

  • Extreme swelling or signs of infection—also to the tissues around the eye/s.
  • Severe photophobia (sensitivity to light).
  • A “thunderclap” headache either with or without any visual symptoms.
  • Getting any chemicals in or around the eyes.
  • Burns in or around the eye/s.
  • Acute glaucoma (symptoms include reddening, pain, visual disturbances, vomiting, or feeling nauseous).
  • Not being able to open one or both eyes.
  • Pupils that are different sizes (and it’s not normal for the person).

This list is by no means exhaustive but gives some good examples of when emergency eye treatment is needed. The ER is open 24/7/365—if you’re able to get a lift or get yourself there safely then you should do so.

2: Call an ambulance

Calling an ambulance would be an option if you’re unable to safely get yourself to a hospital. Many of the reasons already mentioned might be in this category—every emergency is individual and should be assessed as such. An example of when an ambulance might be appropriate is if a large projectile has become embedded in the eye. The paramedics will be able to stabilize the object before moving—plus they can administer strong pain relief and fluids to help combat any excessive blood loss.

The further you are from emergency help and the more serious the injury/issue, the more likely it might be that an ambulance is appropriate. However, this certainly isn’t a hard and fast rule—every situation should be risk-assessed for the best course of action.

3: The nearest eye hospital/clinic/center

While most dedicated eye clinics or hospitals aren’t open 24/7, if your eye emergency falls within the operating hours then heading to a specialist unit could be the right option. This is especially relevant if the hospital has surgical facilities.
